At Limbwise Physiotherapy, we understand that clarity, communication and clinical excellence are essential, especially when supporting complex rehabilitation cases and working within case-managed pathways. Our approach to rehabilitation is proactive and highly collaborative, ensuring seamless coordination with referrers, case managers and multidisciplinary teams. We keep both clients and the wider clinical team fully informed throughout every stage of the rehabilitation process, providing specialist physiotherapy input that is transparent, goal-driven, and tailored to each individual’s recovery journey.

Enquiries & Referral
Referrals to Limbwise Physiotherapy are welcomed from case managers, solicitors, healthcare professionals and private clients seeking specialist rehabilitation following limb loss or complex lower limb trauma. We support clients involved in complex rehabilitation, case-managed pathways and medicolegal processes.​
Enquiries can be made via phone or email and we respond promptly to confirm service suitability, availability, and outline the next steps. A formal letter of instruction is required before proceeding to the initial consultation.​
Our streamlined referral process ensures timely access to personalised rehabilitation for individuals recovering from limb loss or complex lower limb injury.

Initial Consultation & Assessment
Our initial physiotherapy assessment takes place in the client’s home or at an agreed location within their local community, based on location and preference. The initial consultation assessment provides a detailed picture of the individuals current functional ability, medical background, goals and aspirations, with a focus on issues related to limb loss or complex lower limb trauma.​
We consider the person's lifestyle priorities, daily challenges and any physical, psychological or environmental factors that may influence their recovery. This holistic approach ensures each rehabilitation plan is tailored and aligned with the client’s personal circumstances from the very start.

Physiotherapy Initial Needs Report
Following the initial assessment, we provide a clear, structured clinically reasoned physiotherapy report tailored to the client’s individual rehabilitation needs. This includes objective findings, baseline functional measures and a goal-oriented rehabilitation plan designed for those recovering from life changing injuries.​​
The report outlines recommended therapy frequency, potential collaborative input (such as prosthetists or psychologists) and any appropriate onward referrals. Designed to support effective communication across clinical, legal and multidisciplinary teams, our reports ensure all involved in the client's recovery have a transparent understanding of their current ability, aspirations and recommended rehabilitation pathway for the first six months.

Commencing rehabilitation
Once the rehabilitation plan is agreed and funding is approved, we begin delivering physiotherapy in the client’s local community. Our focus is on restoring independence, improving function and building confidence in real-world settings. Each session is client-centred, flexible and tailored to the their goals, whether that’s walking with a prosthesis, returning to work, jogging in the woods or managing to wash the dishes. This community-based approach ensures rehabilitation is relevant, meaningful, and aligned with everyday life after limb loss or lower limb trauma.

Keeping You Informed
We provide clinically reasoned progress reports at intervals of three and five months during the treatment episode, outlining the client’s progress in relation to personal goals, relevant outcome measures and any adjustments or changes to the rehabilitation plan, along with ongoing recommendations. Updates also detail any significant clinical developments or emerging recommendations. Throughout the rehabilitation process, we maintain consistent communication with referrers and the multi-disciplinary teams to ensure shared understanding and coordinated support.

Multi-disciplinary Team Collaboration
At Limbwise Physiotherapy, we actively promote multidisciplinary team (MDT) collaboration to ensure cohesive, client-centred care. We regularly liaise with treating professionals, including prosthetists, occupational therapists and community rehabilitation teams, to align goals, share expertise, and maintain consistency. We contribute to joint planning, shared documentation, and reviews, attending MDT meetings virtually or in person where required. Through collaborative working, we bring specialist insight that strengthens outcomes and supports the wider team in delivering effective, joined-up rehabilitation.
